[MED-GS] Appendicitis & Alvarado score

Background:
This score originally is for evaluation of the nessesar of CT scan for abdominal pain, but now it is really useful for initial differential diagnosis of acute appendicitis.

The Alvarado score
Symptom：　Migratory RLQ pain (1)　Anorexia (1)　Nausea/vomiting (1)
Sign：　Tenderness(RLQ) (2)　　Rebound tendmess(RLQ) (1)　　Elevated temperature (1)
Laboratory：　Leukocytosis (2)　　Shift to the left (1)
Total 10
score>6 for men: highly suspect acute appendicitis
sonogram is recommended for the first choice of diagnosis tool
score>6 for women: should rule out PID EP, and then highly suspect acute appendicitis

[Mood]Will dream come true?

At the time that dream come true, it is the time that dream is broked. When I do realize that being a doctor in Taiwan, what are only left in life are working, working and sleeping. There are 3 holidays a month, but there are 10 days on duty a month. The payment is about 300 euro a month. Nothing to learn, but learn by doing error or asking coworkers.

有三種on port A的規格
(1)大棉棒消毒
(2)大棉棒消毒+治療巾
(3)換藥碗消毒+治療巾

Tegadon+

(1)問病人的portA裝在哪?並用手去確認位子。
(2)問病人上次on PortA有無回血

疑難雜症
有的病人深呼吸的時候會位置改變很大，切記先觀察病人深呼吸一次後再下針
上次有回血而這次沒有回血，或覺得Heparin推得不順，請護士先掛N/S
由於病人血球狀況都不好，re on或是try第二針，就要注意傷口有沒有滲血。

今天考了軍陣醫學後，就一路睡到快四點才起床，一直晃到現在，剛看完韓片雙面女友，撇掉韓片的老套，但我就是會被小小的感動到，算是還不錯的小品。

這個過年前夕一時間突然湧上好多感覺，

這次寒假出國一開始有點半強迫的出國，因為不出國的話，萬一交換又沒去成，就損失了，所以決定寒假一定要衝一趟出國，不過萬萬沒想到這是我和法國第一次的見面，也萬萬沒有想到預算竟然換這麼爆表。

閒聊: Google與我

Google and I (Goole與我)

不知道是甚麼時候我開始用Google的產品，只記得Google一推出我就開始使用了，之後Google如預期的聲名大噪，成為最受歡迎的搜尋引擎，妙的是從此之後我幾乎沒有再離開過Google搜尋。之後就不斷期待尋找Google的產品，(尤其是Gmail等超久)，也買了Google自傳書。最令我著迷於Google的除了極為聰明而強大的搜尋引擎外，更重要的是Google的工作哲學，打破工作和生活的疆界，生活就在工作，工作就在生活中，讓我以為全世界最棒的工作或是最棒的生活就是在Google。

不過還是有些建議要跟Google提一下：
1.Google的意見信箱不太容易進去。
2.Blogger原始版型跟設計比起其他網誌系統實在是簡陋了一點，特殊功能使用難度門檻較高)，但是對於專業的玩家卻是高度自由的平台，不過網誌就是要人多才好玩呀。
3.能不能讓Blogger能認出無名的帳號，這樣就可以讓兩個網誌互相互動提高曝光度使用度和顧客群。

以上只是因為我很想要用blogger當網誌，可是使用上卻不如預期好用，才提出的想法。
